Mudassar Parkhani, PharmD, R.Ph, BSc.Phm, Hon.BSc

Mudassar attained his Pharmacy degree and Doctorate of Pharmacy (PharmD) from the University of Waterloo - School of Pharmacy in 2012 and 2018 respectively. Prior to starting Pharmacy school, Mudassar completed his Honors. Bachelor of Science degree from the University of Toronto in 2008 with a double Major in Human Biology and Zoology. He completed his clinical pharmacy training at the Brantford General Hospital in Ontario. He is certified in motivational interviewing, smoking cessation, and immunizations. Moreover, he is working towards obtaining his travel health certification. 

Health is a continuum that not only involves diagnosis and treatment, but also health promotion and prevention. Mudassar brings a plethora of knowledge and experiences from different pharmacy practice settings including hospital, industry, and community pharmacy alike. He has helped implement lean principles and stream line pharmacy operations at over 30 stores at Loblaw Pharmacy.  He has a keen interest in primary prevention, identifying gaps in patient therapy, and optimizing individualized medication regimens for his patients. He is actively involved in the design and implementation of evidence-based screening and treatment programs for a variety of disease states. 

Mudassar is an OSCE Assessor with PEBC (Pharmacy Examining Board of Canada) and is part of the admissions team at University of Waterloo - School of Pharmacy to screen and interview prospective students applying to the School of Pharmacy. He is an active member of the Ontario College of Pharmacist (OCP) and Ontario Pharmacists Association (OPA).

Shane Dias, BSc.Phm, RPh

Shane graduated with a degree in Pharmacy from the University of Waterloo (2012) after completing his Bachelor of Science from the University of Toronto (2008). He is looking to complete his Doctor of Pharmacy from the University of Waterloo over the next couple of years. His practice expertise includes drug information, drug utilization review, medication therapy management and geriatrics. His focus has been cost effective medication management programs in a collaborative group setting, including target drug programs for providers and individual patient consults to minimize out-of-pocket medication expenses. He is excited to be able to work as part of our team and to be able contribute to positive patient outcomes at a primary care setting.
